Q: What is in the WBW Kit? - OR - Where can I find the Forms I need?

A: All the forms you will need to host a WBW event, plus any others you could find useful in organizing and/or administering your WBW event are in the WBW Kit.

For AWCs only - REQUIRED

AWC ACCOUNTING FORM - Microsoft EXCEL
Self-tabulating spreadsheet.

AWC ACCOUNTING FORM - Microsoft WORD
Same form as self-tabulating - you write in details and calculate totals yourself.

DOLLAR DONATION FLYER
Colorful flowers: write in name of donor.

DONATION BROCHURE
Solicitation brochure to mail or deliver in person. Can be personalized with your name or Group info.

Use this form to sign-in attendees at your event and to keep a list of who attended. By signing the form, attendees agree to: permit LLL to use their photograph, a waiver (legal disclaimer for liability) for event.

Lactation Luncheon, (n), 1. An event that brings together members of the breastfeeding community, in order to share ideas, forge bonds and discuss future cooperative ventures.  May be hosted by a hospital, health clinic, task force, civic health centers, or community volunteers.  2. Could also be a breakfast, dinner or tea.  It can be scheduled any time of the year, or as a celebration during World Breastfeeding Week in August.
