Q: Which forms should I use? Are any forms mandatory?
A: Only the Accounting/Prize
Eligibility/Evaluation Form is mandatory. You may find other forms useful.
You can also personalize the forms to fit your event, add the name of your Group,
etc. Fill out the Accounting/Prize
Eligibility/Evaluation and mail it to the AWC, postmarked by October 17,
2008. If anyone is eligible for prizes, be sure the Prize Eligibility part of
the form is filled out. If anyone donated $300 or more, return the Donor Incentive Flyer so they
will be recognized in NEW BEGINNINGS and receive their prize if eligible. See
