2009 Online Chapter Registration Form

Chapters should register only if all the proceeds go to a Chapter treasury. If not, register each Group separately. Although there is NO FEE for either Group or Chapter registration, you must register to be eligible for prizes or to receive a site insurance certificate. Please help us keep an accurate count by listing all the participating Groups in your Chapter.

You may register online here or mail this information to Pam Dunne, WBW Registrar, 7295 Highland Estates Place, Falls Church, VA 22043 (use one of the printable forms for this purpose). Do not register as a Chapter unless all the proceeds go to a Chapter treasury.

Events may be held from May 1 - Sept 30, 2009. Registered Groups may send the insurance request form two weeks prior to the event date if your event site needs a certificate of insurance. Send the form with the request to the insurance company.

To be eligible for prizes, money and paperwork from registered Groups must be postmarked to their AWCs by October 17, 2009. DO NOT send money directly to LLLI.
